About the Practice

Diadem Medical Practice is a large teaching practice serving patients in the Bilton Grange and East Hull area. Since 2010, we practice from new purpose built premises on the ground floor at Bilton Grange Health Centre, providing easy access for our disabled patients.

Bilton Grange Health Centre has good transport links, with bus stops outside the premises. Our large, airy waiting room includes a play area for children, baby changing facilities and toilets. There is a lift to the first floor, where community clinics and services are held for both our own patients and those in the locality.

Practice merger with Modality

We are very pleased to inform you that our Practice will be joining with 4 other local practices who currently form a Hull division of the Modality Super Partnership. This formal merger will be effective from 1st July 2017.

The Modality Partnership is committed to the values of the NHS and providing excellent patient care. They currently provide services to 100,000 patients across Sandwell, Birmingham and Walsall.

The practices we will be joining in the Hull Division are Faith House Surgery, New Hall Surgery, Newland Group Medical Practice and Springhead Medical Centre. By joining with the practices we can take advantage of working in a bigger organisation, such as developing new services and improving access. This will also help in making our services more resilient in a challenging time for the NHS.

Please be assured that this change will not adversely impact the level of service and care we currently provide. We plan to share more details of the new organisation and there will be information sessions in the near future to enable this.
